Provides direct services to Tenants of Jasmine Court (759-767 East 138th Street, Bronx, NY 10456) who are single adults who are seriously and persistently mentally ill and substance abusers. Assist tenants in increasing their capacity to live as independently as possible and become an active part of the Jasmine Court community. Some weekend hours required. Typical Duties: • Assist in screening prospective tenants. • Train tenants in activities of daily living, providing restorative services for those in need of them. • Monitor tenant day to day progress. • Work closely with Case Managers and Social Worker. • Assist tenants in accessing community resources and social services, and facilitate services appropriate to the tenants’ needs. • Participate in regular staff meetings, appropriate case conferences and in-service training as well as individual supervisory meetings. Qualifications: High School Diploma, or equivalent, plus experience working with a severely and persistently mentally ill and/or MICA population. Knowledge of Microsoft Office a plus. POSITION: Substance Abuse Counselor Responsibilities: Reports to the Aurora MICA Program Supervisor or designee. Provide direct services to program consumers, who are Severely and Persistently Mentally Ill individuals with serious substance abuse issues. Assist consumers in increasing their capacity to live as independently as possible and become an active part of the wider community. Some evening and weekend hours will be required. Provides driving services for the Heritage Caring Community Program located at our Harmony Court facility at 1305 Morris Avenue in the Bronx as well as for our administrative office at 416 West 127th Street in Manhattan, and to other programs and locations, as needed and assigned. This will require transporting clients, staff and material necessary for the proper functioning of this agency. Some evening and weekend hours may be required.
