     The Heritage HealthCare Center for Behavioral and Mental Health team understands that everyone’s path to wellness looks different as they seek informative and healthy ways to mend and restore a healthy well-balanced life.  The Heritage HealthCare Center for Behavioral and Mental Health believes in demonstrating empathy towards individuals with unique backgrounds who seek out care and works consistently with patients and their families to find their path to a healthy and well-balanced lifestyle.  Our Behavioral Health team offers individual and group counseling to support your emotional and physical well-being while working with you to determine your health and recovery goals while developing treatment plans that are tailored to your specific needs.

Heritage Healthcare Center also has a special focus on our senior citizens. During the pandemic, mane became isolated and disconnected from their family and friends. Isolation leads to depression, anxiety and a decline in cognitive abilities. The Behavioral Health and medical providers at Heritage HealthCare Center are especially sensitive to these issues, and can help. Psychotherapy with one of the social workers can help alleviate the sense of isolation, and help our older patients reengage with family and friends.

     At Heritage HealthCare Center, there is a concerted effort to integrate medical care with behavioral health whenever issues of grief, addiction, depression, anxiety or other personal stressors occur. This holistic approach as well as working collaboratively with our multidisciplinary team helps ensure all aspects of patient care are met.  Heritage HealthCare Center believes that a mind and body approach is pivotal for optimal health.  Having   General Medicine, Behavioral and Mental Health clinicians and Psychiatrists on site helps to facilitate the process in which individual counseling, medical care and psychiatric care work seamlessly together.

Adolfo is a bilingual (Spanish) licensed clinical social worker that provides psychotherapy to
individuals, children, adolescents, and families. Adolfo has significant experience working with outpatient mental health clinics serving underserved and minority communities.

 

Adolfo oversees the provision of behavioral and mental health services at Heritage and works closely with the Chief of Psychiatry, Chief of Medical affairs, medical and dental providers, and other psychiatric providers to develop quality, comprehensive, and integrated services through a patient centered approach. Adolfo is interested and excited to work with the patients at Heritage Health and with the multidisciplinary team at Heritage.

psychotherapy with adults working both at a PROS program for the elderly as well as outpatient adult mental health clinics.  Sonya is well versed using evidenced based practices such as DBT (dialectical behavioral therapy), CBT (cognitive behavioral therapy) and trauma based modalities to yield optimal treatment results.  Having over 30 years of social work experience, Ms. Prystajko brings a deep passion and commitment to her work forging a professional relationship with her clients directed at inspiring hope, personal growth and healing.

     Sonya provides individual therapy, conducts psychosocial/psychiatric assessments, psychoeducation and strives to help patients overcome mental health barriers/substance abuse disorder that interfere with their health care and personal goals.  Ms. Sonya Prystajko is also active in advocating for her patients and assisting them with case management services when necessary.  Ms. Sonya Prystajko is delighted to be part of the Heritage family and to serve the Harlem Community where she originally began as a clinician while working for Upper Manhattan Mental Health Center.

      Angelica provides bilingual insight oriented psychotherapy for individuals, children, adolescents, adults, families, couples, and groups, utilizing a multimodal approach that is individualized for each patient. Angelica utilizes a strength based eclectic approach in her therapy. Treatment modalities include: Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, Mindfulness, Motivational Interviewing, Trauma Focused Therapy, Holistic Health/Wellness Based, Play Therapy, Harm Reduction and Relapse Prevention. 

 

     Prior to joining the Heritage Health family, Angelica’s work experience includes the Jewish Board for Family Services, Union Settlement Association, Services for the Underserved, Grand St. Settlement House, and East Harlem Health Council. Angelica is a NASW member and is a graduate of the Foundations in Family Therapy training at The Ackerman Institute of Family Therapy. Angelica is a pioneer member of the Sweet Institute and is currently receiving training in EMDR.

    We serve an extraordinary need in Harlem. Heritage is committed to addressing the
mental and behavioral health needs of our community. Our commitment to our
community is demonstrated by how we have grown our practice over the past few
years. Most facilities take weeks to schedule appointments. At Heritage, we can get
you an appointment within 24 hours. Our team is continuing to grow and is here to
serve the Harlem community. We are available to provide you with appointments for
therapy as quickly as we can.


    At Heritage, we provide mental health counseling to adults, children, and families. Our
team works with your medical providers in a collaborative manner that is patient
centered. We offer individualized treatment to help adults and children reach their
mental health and physical health goals.


    Our team of psychiatrists, and psychiatric nurse practitioners, social workers, social work interns,
work with our patients to provide comprehensive assessments, mental health screenings, individual counseling, psychiatric evaluations and medication
management. We treat a wide range of conditions including depression, anxiety, and
difficulties adjusting to medical conditions. We offer visits in person and virtually.
Heritage is committed to providing individuals with the best mental health support
services possible. We offer training experience to graduate students in social work who
can work closely with our students. We have provided fieldwork opportunities to
students from Fordham University, Hunter College, and Columbia University. Each year
student interns are available for appointments between September and May.

Adolfo Marrero, LCSW

Adolfo Marrero, LCSW, is the Director of Heritage HealthCare Center for Behavioral and Mental Health. He is committed to providing integrated behavioral and mental health services to the patients of Heritage Health and the Harlem/Hamilton Heights community. Adolfo has many years of experience working as the director of a federally qualified health center in Harlem, developing behavioral health care management programs from the ground up. Adolfo also has significant experience in implementing behavioral health integration programs through various funding opportunities.
 

Sonya Prystajko, Behavioral and Mental Health Clinician, LCSW

 

Sonya Prystajko graduated from Yeshiva University New York, NY in 1997 with a Masters in Social Work and is SPIFI certified through Hunter College to supervise graduate students.

 

Sonya has an extensive history working with the geriatric population including elderly Holocaust survivors providing case management, mental health outreach and supportive therapy. More recently, Sonya began specializing in

Angelica Perricone, MSW, LMSW

 

     Angelica Perricone, MSW, LMSW is a licensed social worker dedicated to serving our Harlem and Washington Heights communities.

 

     Angelica received her Master’s Degree in Social Work with honors from the New York University Silver School of Social Work, and graduated with her Bachelors of Science in Psychology, Cum Laude from Marymount College.
