Heritage Healthcare Center for Crohn's, Ulcerative Colitis and Liver Disease

     The Heritage HealthCare Center provides specialty care to patients with complex chronic inflammatory gastrointestinal diseases, chronic viral hepatitis and fatty liver disease.

 

     It is our mission to offer high-level concierge care for all gastrointestinal diseases, with a focus on chronic viral hepatitis, Crohn’s Disease and Ulcerative Colitis.

Our providers, Lincoln Hernandez, M.D. and Dominique White F.N.P. create a highly personalized, multidisciplinary care plan for our integrated services that include:

 

  • Initial consultation

  • Risk profile for possible effects on the course of your IBD or potential complications

  • Review of current medications for compatibility with IBD treatment

  • Annual preventive health assessments

  • Ongoing disease monitoring

Dr. Lincoln Hernandez

     Dr. Lincoln Hernandez is a gastroenterology specialist practicing in New York City since 2012. Dr. Hernandez completed his internal medicine residency at NYU Downtown Hospital with top honors earning the Program Director’s Award and the John T. Flynn Young Investigator Award. 

 

     This was followed by fellowship in Clinical Nutrition at New York-Presbyterian Hospital/Columbia University Medical Center. During this time, Dr. Hernandez served as Instructor in Clinical Medicine and subsequently became a research fellow at Columbia University's Celiac Disease Center successfully publishing his work in Celiac Disease and other peer review journals.

 

     Dr. Hernandez completed his fellowship training in Gastroenterology & Hepatology at Tulane University's School of Medicine in New Orleans.  Dr. Hernandez was then appointed as Gastroenterologist for Ochsner Clinic, Baton Rouge. Here Dr. Hernandez excelled as a young specialist and contributed to the expansion of gastroenterology clinics and performing complex therapeutic endoscopy.  In early 2012 he returned to New York City where he currently practices.

 

     Dr. Hernandez’s focus includes Inflammatory Bowel Disease including Crohn’s and Ulcerative Colitis, Chronic Liver Diseases such as Fatty Liver, Hepatitis C and Hepatitis B and Gastric Reflux Disease and Colon Cancer Prevention.  Dr. Hernandez holds a faculty position of Assistant Clinical Professor at the Icahn School of Medicine of the Mount Sinai Hospital where he teaches new fellows’ endoscopy and precepts their community clinics.

 

     Dr. Hernandez has dedicated much of his career to the delivery of specialized medicine to the Central Harlem community and most recently is coordinating efforts in the new Heritage HealthCare Center for Crohn’s Colitis and Liver Disease focusing on chronic Gastrointestinal and liver disease care.
